Description Participants that have advanced stages of certain chronic illnesses, such as CHF, COPD, PVD, end-stage renal disease, neuro-degenerative diseases and/or advanced malignancies will be considered for this study. Patients will complete questionnaires to compare outcomes of care between patients who are part of the OACIS program and those who are not. Participants who are not part of the OACIS services will receive questionnaires to complete and return to the primary investigator. By completing this questionnaire, consent is authorized.
EligibilityInclusion Criteria-including but may not be limited to:
o Age 55 and older
o Have reached advanced stages of certain chronic diseases
o Have been hospitalized within the past 18 months at least one time for their disease
Purpose This study is to look at patient outcomes for participants in the OACIS program vs those who are not involved in the program.
