HEALTH CARE SYSTEMS FOR INCREASING AND TRACKING COLORECTAL CANCER SCREENING TESTS: PHASE 2

This study will entail focus groups with both patients and doctors practices. There will be two patient focus group sessions, broken down into 5 individual focus groups per session. One of the focus groups will be held at Lehigh Valley Hospital, Cedar Crest site and the other one will be held at the LVH-Muhlenberg site. The participants will discuss their response to various hypothetical situations relative to the information process and intervention by physician groups regarding colorectal screenings.
- Diabetes Education:
USING A COMMUNITY HEALTH WORKER (PROMOTORA) TO IMPROVE DIABETES SELF-MANAGEMENT TRAINING IN LATINOS: A COMPARATIVE APPROACH (Investigator Initiated Protocol)

This study is a comparison of two pre-existing Spanish speaking diabetes education programs. Participants already part of the Promotora education courses will be asked to consent to participate in this research study. Participants in the study will be asked to complete additional questionnaires at 6, 12 and 18 months after the completion of the class. A focus group may take place during the 6, 12 and 18 month questionnaire sessions to discuss any issues. Participants may ask to meet privately with the study team at any time.

COMPARATIVE EVALUATION OF PROGRAMS TO IMPROVE THE CARE PATIENTS WITH ADVANCED
COMPLEX ILLNESS (OACIS AND 3CI) PART 1: EVALUATION OF AN INTERVENTION AT THE PRIMARY CARE OUTPATIENT LEVEL

Participants that have advanced stages of certain chronic illnesses, such as CHF, COPD, PVD, end-stage renal disease, neuro-degenerative diseases and/or advanced malignancies will be considered for this study. Patients will complete questionnaires to compare outcomes of care between patients who are part of the OACIS program and those who are not. Participants who are not part of the OACIS services will receive questionnaires to complete and return to the primary investigator. By completing this questionnaire, consent is authorized.
- Insomnia -
A PILOT STUDY TO ASSESS THE IMPACT OF MINDFULNESS-BASED STRESS REDUCTION ON INSOMNIA IN A PRIMARY CARE PATIENT POPULATION

This study is for patients who have been diagnosed with sleep disturbances and anxiety, for the treatment of insomnia. Potential participants will be asked to contact the Lehigh Valley Hospital Sleep Center and be screened by telephone. Patients diagnosed with primary insomnia will then be enrolled in the study. Participants will attend an information session which will provide details for upcoming classes which will be held once a week for 8 weeks. Participants will need to complete questionnaires and surveys; and be able to maintain a sleep diary. Evaluations will need to be completed at each class.
